I applied for a portion of the $20,000,000 class action refund that Quaker
State is rebating to purchasers of the Slick 50 additive. There was an
application in our Sunday paper "Parade" supplement. They rebate $15 per
quart of Slick 50 purchased, and it is good for Quaker State products and
services. IIRC, if the original $20,000,000 is not distributed, they will
do a secondary distribution to the participants of the class action.

You could get a single rebate by completing the form, with no proof of
purchase required. Multiple rebates required proof of purchase.
